Forms

Below are the forms currently used by the guild for member activities and administrative purposes. Click the links to view, download, or print each form as needed.

To initiate or renew your membership, please print the Membership Form, complete it, and bring it to the next guild meeting or mail it with your check for the membership fee.


If you have questions about membership, feel free to email the Membership Committee.

The Guild is incorporated as a non-profit public benefit corporation and is exempt from federal income tax under Section 501(c)(3) of the Internal Revenue Code. Items donated to the guild—such as quilts, fabric, notions, and supplies—may be reported as charitable contributions for federal tax purposes.
Please use the Donation Form to document your contributions for your records.

If you’ve purchased items for the guild with your own money, you may be reimbursed for the expense. To do so, print the Guild Expense Reimbursement Form, complete it, and attach all receipts to substantiate your expense. Please submit the form and receipts to the guild treasurer for processing.

If you are donating items to the auction for the quilt show, please print the Auction Donation Form, complete it, and include it with the item being donated. This helps us keep track of all donations and ensures that we properly acknowledge your generous contribution.

If you need to submit money to the guild outside of regular meetings, please print the Guild Income Submission, complete it, and include it with the money you are submitting. This helps us keep accurate records of all incoming funds.

During quilt show activities, you may need to obtain reimbursement for expenses related to the committee you are supporting. Print the Quilt Show Expense Reimbursement, complete it, and attach all receipts to substantiate your expense. Please submit the form to the Quilt Show Accounting Chair for processing.

Use this form to submit your quilt for display in our upcoming quilt show. All guild members are encouraged to participate—share your creativity and be part of this special event! One form is required per quilt entry.
